Accessing Medical Records
Patients seeking their medical records can now access them in person again effective Monday, November 1, 2021. Like other visitors to the hospital, you must have proof of a COVID-19 vaccination or a negative COVID test within the past 72 hours to enter the hospital.
To reduce the amount of time you must wait for your medical records, we ask that you call the HIM department in advance of coming to the hospital to set up an appointment. By making an appointment, we can process the request and have records ready when you arrive at HIM. If you arrive at the hospital without an appointment, you can still access your medical records in the HIM department as long as you meet the requirements to enter the hospital, e.g., proof of COVID vaccination/negative COVID testing requirement.

Once in the HIM department, you will be required to complete the medical records release form before being provided copies of their medical records. Patients who do not meet the vaccination/testing requirement will still use the electronic medical records request process.
HIM hours of operation are Monday through Friday: 8:00 am – 4:00 pm (closed on holidays).
You are still able to request your medical records by downloading this Release of Information Form, filling it out and doing one of the following:
